Required services:      Consultancy to Provide Key Population Services

Location: OSS, Taraba, Nigeria

Duration: 27 Days with Possibility of extension

Commencement Date: 25th Nov, 2021

Proposed Dates: Between 25th Nov – 31st Dec, 2021 (excluding Public Holidays)

Background

Achieving Health Nigeria Initiative (AHNi) is a sub-recipient to FHI 360 on the “National Aligned HIV/AIDS Initiative (NAHI) project. Under this arrangement, AHNi has a mandate to support the provision of both facility-based and community-based comprehensive HIV services to the key population (KP) community in Taraba state. This support includes the provision of integrated community-based one stop shop clinics (OSS) and key population friendly health services (KPFHS) through faith-based, private, and public health facilities. The OSS clinic model is a type of community-based differentiated ART service delivery model for KP. The KPFHF is aimed at sustainability and ownership of KP care through provision of ART services to KP in a conventional healthcare setting. The OSS will have full continuum of staff (Front Desk Officer, clinical officer, Pharmacist, Triage Nurse, Laboratory Scientist, Psychologist and Adherence counsellor, Data entry clerk, case managers, janitor, and security personnel).

The OSS will be a Safe haven to members of the key populations community as it will provide non-biased, non-stigmatizing and nonjudgmental friendly services to the designated populations.  The OSS will be opened to members of the KP community for all 5 working days of the week from 8:00am to 5:00pm on daily basis. However, there will be context specific service provision utilizing extended hours of service provision to address the need of clients who may not be disposed to visit the OSS during working hours or working days as arrangements can be made to visit at weekends based on discussions with the service providers at the facility.

The following medical personnel who will work as consultants are required in the OSS as stated below in details.

The specific objectives of the KP intervention include:

  • To create a supportive environment for key populations and foster community empowerment by providing a venue for relaxation, recreation, peer support, income generation and other activities to address underlying social factors such as SGBV and social marginalization. 
  • To provide health education and promote safer behaviors (including safer sex and safe injection) to reduce the risk of acquiring or transmitting HIV and STIs.
  • To provide an integrated package of HIV/STI/RH services tailored to the needs of the KP clients through ‘friendly’ service providers who are sensitized and trained to serve KP.
  • To serve as a distribution point for condoms, lubricants and needles/syringes and other key commodities to reduce HIV transmission.
  • To build the capacity of KP CBOs to implement comprehensive HIV prevention and health promotion interventions.

The positions to be filled include:

Basic Function: The main responsibility will be to provide education about HIV-related issues and adherence counseling services at One-Stop-Shop (OSS) for Key Populations (KPs) and their partners.

TRIAGE NURSE (x1)

Basic Function: The main responsibility will be to provide education about HIV-related issues and adherence counseling services at One-Stop-Shop (OSS) for Key Populations (KPs) and their partners.

Specific Deliverables are:

  • Provide psychosocial and continuous support as well as treatment/PrEP preparation to KPs and their partners in the OSS.
  • Identify and address challenges/barriers to adherence to care and treatment to ART for KPs and PrEP for their partners.
  • Organize and facilitate support group sessions for infected and affected KPs.
  • Address aspects of behavior change in relation to HIV and ART.
  • Conduct Sexual network tracking/Partner notification services.
  • Facilitate the delivery of PWP information to clients enrolled in OSS.
  • Participate in home visits as appropriate to execute plan of care for clients.
  • Ensure client records are well maintained and compile periodic reports as necessary.
  • Ensure proper referral of patients where necessary including following up and reporting on the outcomes as necessary.
  • Coordinates patient tracking activities supported by the Case Managers including documentation and reporting of defaulter tracing activities.

Reporting

  • Reports to the facility in charge and GF Technical leads.
  • Works in collaboration and guidance of the Clinical Technical team.
  • Compile and submits monthly reports on Psychosocial Support and Patient tracking activities.

Requirements:

  • BSC Nursing/Nursing Midwife
  • Nurse Educator
  • Diploma in Counseling Psychology

Experience & Skills:

  • 3 – 4 years’ experience, 1 year of which should be as an Adherence Counselor in HIV service delivery.
  • Excellent interpersonal and communication skills
  • Computer literacy

FRONT DESK OFFICER (x1)

Basic Function: He/she is the first point of contact with the KPs or any other person entering the OSS. He/She must be a KP community member.

  • Greet all the entrant into the OSS.
  • Register clients according to established protocols.
  • Assist Clients to complete all necessary forms and documentation.
  • Ensure Clients information is accurate.
  • inform Clients of medical office procedures and policy.
  • In liaison with the record unit maintain and manage client records
  • move Clients through appointments as scheduled.
  • Answer incoming calls and deal with inquiries.
  • Transfer calls as required.
  • Schedule Clients appointments
  • Deal with incoming and outgoing post
  • complete other clerical duties as assigned.
  • maintain stock of forms and office supplies.
  • ensure reception area is well maintained, neat and clean.
  • safeguard Client’s privacy and confidentiality

Qualifications:

BSC/ND/HND administrations/Secretarial studies.

3-5 cognate experience in a similar or related field.

LABORATORY SCIENTIST (x1)

Basic Function: The main responsibility will be to provide Laboratory services to members of the Key populations and their partners at One-Stop-Shop (OSS).

Specific Deliverables are:

Area 1:   Provide laboratory and point-of-care diagnostic services in support of HTS and ART for Key Populations.

  • Performing laboratory tests to produce reliable and precise data to support provision of the continuum of HIV treatment services.
  • Preparing Dried Tube Specimen (DTS) panels for IQC and EQA (Proficiency testing) to support HTS on the project.
  • Carrying out routine tasks accurately and following strict methodologies to carry out analyses.
  • Preparing specimens and samples
  • Maintaining and operating standard laboratory equipment
  • Maintaining personal expertise in clinical laboratory practice and innovative technology and equipment
  • Ensuring the laboratory is well-stocked with relevant consumables.
  • Following and ensuring strict safety procedures and safety checks within and around the laboratory

Area 2: Oversight and Supervision of Laboratory Services

  • Working with the project team to develop and implement quality assurance systems to improve the quality of diagnostics and facilitate treatment and monitoring of HIV.
  • Supporting the development of project work plans and budgets to incorporate the procurement of equipment and supplies for laboratory services.
  • Supporting the use of emerging and appropriate technology for cost-effective service delivery
  • Supporting the design and implementation of innovative strategies for delivering needs-based training and mentoring for clinic staff in relevant areas
  • Supporting implementation and monitoring of biosafety and biosecurity in clinical service delivery, including the development of SOPs, guidelines and job aids

Area 3: Data Management and Reporting

  • Produce computerized records to interpret and archive laboratory results to be presented to clinic staff and project team for informed decision making.
  • Provide support to the Strategic Information (SI) unit to support research (qualitative and quantitative) surveys on service delivery outcomes for HIV prevention, care and treatment.
  • Ensure timely submission of quality reports to the PR/SR on monthly and quarterly basis or as required.

Area 4: Other assignments

Proactively seek out and report relevant opportunities, success stories, linkages or challenges that may promote laboratory services for the project.

  • Participate in Laboratory Technical Working Groups and represent the project at relevant fora.
  • Carry out any assignment as may be decided by the Clinical Supervisor, PR/SR

Qualifications:

  • AMLSC with at least 5 years of progressive experience with a GF or PEPFAR supported laboratories.

PHARMACIST (x1)

Basic Function: The main responsibility will be to provide pharmaceutical care services and support medication adherence counseling services at One-Stop-Shop (OSS) for Key Populations (KPs) and their partners.

Specific Deliverables are:

Area 1:   Delivery of pharmaceutical services in support of quality Antiretroviral Therapy (ART)

  • Dispense Antiretroviral drugs (ARVs) and other medication in line with approved regimens and standards.
  • Dispensing ARVs in appropriate packaging with clear instructions
  • Providing medication-related counseling, including reinforcing adherence and advising on potential side effects and drug interactions
  • Providing drug information to health service providers and clients on request
  • Identifying and supporting follow-up of clients who do not return for refills of their ARVs.
  • Working with providers and clients to identify, manage, and report adverse drug reactions and drug interactions.
  • Contributing to decision making on treatment and care for individual clients
  • Ensure Good Pharmaceutical Practice (GPP) and standards
  • Calculate drug storage needs for ART start-up and expansion.
  • Monitor ARV and other drug (STIs, OIs, etc.) stock levels at the OSS and communicate with the project team for re-stocking.
  • Contribute to forecasting /quantification of health commodities and monitoring of drug distributions.
  • Oversee drug logistics i.e., stock-taking, proper drug storage, drug stock out and monitor the effectiveness of the pull system.

Area 2: Internal and External Quality Control Systems

  • Assess the functionality of existing pharmaceutical management systems and develop implementation plans that prioritize and address gaps.
  • Oversee quality assurance systems at the OSS by close monitoring of prescription patterns, ensuring proper documentation, side effects monitoring (pharmacovigilance) and reducing dispensing errors.
  • Identify needs for information resources—for example, ART guidelines and drug information for medication counselling.

Area 3:  Monitoring and Evaluation

  • Implement monitoring and reporting of medication errors, suspected adverse reactions to medicines, and other medication-related events.
  • Monitoring ART prescriptions for appropriateness and discrepancies
  • Monitoring adherence to ARVs; working with clients and other health service providers to address difficulties.
  • Address emerging problems as client numbers increase.
  • Contribute to the development of data collection tools for KPs (HTS, ART and Prevention for Positives)
  • Monitor and report weekly enrolment.
  • Provide support to the Strategic Information (SI) unit to conduct research (qualitative and quantitative) surveys on service delivery outcomes for HIV prevention, care and treatment.
  • Ensure timely submission of quality reports to the PR/SR on monthly and quarterly basis or as required.
  • Maintenance of transaction records and inventory management tools as required to ensuring transparency in commodity management records.
  • Ensure timely, accurate and timely reporting of logistics reports to facilitate stock replenishment and accountability.

Area 4:  Other assignments

  • Proactively seek out and report relevant opportunities, success stories, linkages or challenges that may promote clinical services for the project.
  • Carry out any assignment as may be decided by the PR/SR.

Minimum Recruitment Standard:

  1. B.Pharm or Pharm. D with at least 2 years minimum Pharmaceutical experience and working with Key Populations
  2. A registered member of the pharmaceutical council of Nigeria will be a huge advantage with valid council registration
  3. Experience working with Civil Society Organizations
  4. Excellent ability to communicate and maintain diplomatic and productive relations with the team; resourcefulness, flexibility, and ability to work in a fast changing and challenging environment
  5. Familiarity with Nigeria Public Sector Health Systems, NGOs and CBOs will be highly desirable

PSYCHOLOGIST (x1)

The Psychologist will work under the supervision of the OSS Clinician and PCT lead to provide Clinical Psychology support through the performance of the following duties: 

Specific Deliverables are:

  • Work within a multidisciplinary team in the OSS center according to the case management, bio-psychosocial approach.
  • Conduct screening and assessment of beneficiaries and develop individualized treatment and rehabilitation plans according to their needs.
  • Provide psychoeducation to beneficiaries and their families.
  • Provide psychological support and counseling services to beneficiaries and their families.
  • Provide family support when needed.
  • Facilitate group therapy and support group sessions in coordination with the multidisciplinary team.
  • Conduct home visits to provide further psycho-social support when needed.
  • Organize community, rehabilitation and awareness activities for beneficiaries and their families in coordination with the multidisciplinary team.
  • Attend and contribute to the weekly clinical supervision meeting.
  • Ensure the complete and updated filing of beneficiary information, including follow-up visits and referrals.
  • Assist in performing other tasks as requested. 

Minimum Recruitment Standards:

  • BA/BSc in Psychology (MA degree is preferred). 
  • At least 2 years’ experience working within a multidisciplinary team. 
  • Previous training on case management and/or bio-psychosocial approach is an asset.
  • Experience in Psychological First Aid, and other psychosocial support (MHPSS) activities
  • Experience in working with people with mental health issues as an added advantage.
  • Fluent in English (speaking, reading, writing).
  • Working knowledge of Hausa is an advantage.

DATA CLERK (x1)

Specific Deliverables are:

  • Design, implementation, monitoring, evaluation and reporting at the OSS.
  • Ensure rigor and quality in data collection.
  • Responsible for data collation, processing and reporting of monthly summary forms from the One Stop Shop (OSS).
  • Carry out data verification exercises at various service points.
  • Participate in program review meetings.
  • Conduct routine Data Quality Assurance exercise at the OSS.
  • Participate in project baseline, midterm and end of project evaluation and special data requests.
  • Maintain an inventory of data collection tools in the facility.
  • Conversant in GF indicators reporting on HIV Prevention, Care and Treatment
  • Carry out other activities assigned.

Education and/or Experience

  • To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or competency required.
  • First degree in social science, public health, or another relevant discipline.
  • At least 1 years M&E experience in the health sector, experience working on HIV treatment program or sexual and reproductive is an advantage.
  • Demonstrate computer literacy in MS-Office- Excel, Power Point and Word

Preferred Skills:

  • A good understanding of HIV Prevention Care and Treatment
  • A good understanding of LAMIS database
  • Proficiency in MS- office MS-Excel, Word, Power Point
  • Ability to multi-task and work under pressure
